Open 7AM -7PM 7 days a week providing same day complimentary assessmentsOnce a referral is received, the BasePoint team promptly contacts the client and their family the same day to learn more about their needs, answer questions, and begin building trust from the very first conversation. Our goal is to schedule an assessment the same day we get a referral.
When a client begins care, our team reaches out to the established outpatient therapist or psychiatrist within 24 hours to inform them of the admission and share any relevant clinical information. This ensures continuity of care and collaboration with the client’s existing providers as treatment begins. We can also facilitate an initial consultation call between providers as needed/requested, but typically Clinical Care Coordinators will fulfill this through our initial contact process.
Referring providers receive a progress update midway through treatment. These updates are designed to keep you informed and engaged in your client’s care journey, ensuring transparency and partnership.
Updates are typically shared by fax or email, though consultative phone calls are available when appropriate. More frequent updates can be provided upon request or when clinically indicated to best support a client’s care.
Discharge planning truly starts at day of admission. We meet with clients/families on admission day as well as call any established providers within 24 hours of admission.
BasePoint’s goal is always to return clients to their primary provider after treatment, with updated treatment notes, recommendations, and continuity plans.
Providers can submit referrals online through our secure form or connect with us directly by phone. Our goal is to make the process fast, easy, and accessible.
